Step 1. Update DEERs:  Ensure you update your orders; your local work and home addresses, your phone and email addresses are also current. If DEERs does not have the best contact information for you and your family, it will delay your medical registration. DEERs office is located at building 5700, DEERs/ID Card section, call 800-538-9552 or https://milconnect.dmdc.osd.mil/milconnect

Step 2. Assign Provider/Primary Care Manager (PCM):  Call TRICARE at 800-444-5445, option 1, listen to Privacy Act Announcement, you will be asked “how can I help you” state I would like a new Primary Care Manager or PCM, option 1, Enter SSN or DOD ID, birthdate, a TRICARE representative will answer. All Active Duty, including “temp AD orders” will be assigned to Team Respect, using last 2 of your last 4 (SSN):  00-25=MAJ Ball, L; 26-50=Ms. Johnson, E.; 51-75=Mr. Sallis, W.; 76-99=Mr. Sullivan, S. If your dependents relocated with you have them assigned too. TRICARE Prime Beneficiaries will be assigned to Team Courage or Team Integrity at LAHC.
 

Step 3. TRICARE Online (TOL):  https://www.tricareonline.com; you must log in to create or update your account Profile and ensure your Military Treatment Facility (MTF) is AHC Lyster-Rucker.  TOL is 24 hours, 7 days a week access to your medical care. Make your own appointments, virtual, same day and future, request medication refills, view lab results, x-ray reports and so much more.

Step 4. Secure Messaging:  https://app.tolsecuremessaging.com communicate with your Provider/PCM and medical team, using secure messaging.  Secure email to your provider to discuss medical, administrative concerns, upload documents and photos for additional medical consultations. You must sign in or register an account; update your Provider and Account information. For issues or concerns with log in or registration, please contact the Secure Messaging support team directly, 24 hours a day at 866-309-4138.

Step 5. MEDPROs:  Call 334-255-9916; Monday-Friday, 0730 -1200 & 1300 -1615; during clinic hours (see clinic hours below). If no answer, please send email to usarmy.rucker.medcom-lahc.list.mcd-virtual-in-out-processing@mail.mil requesting In Processing slides to virtually check your MEDPROs.

Step 6. Behavioral Health, SUDCC and FAP, TRICARE, EFMP and Patient Administration:  These sections will complete your clinic in processing virtually, after you complete Steps 1-5. A LAHC team member will be at bldg. 5700/MPD each duty day by 0745 to assist you with a medical brief and how to sign your in processing packets. NOTE:  Please do not come to Lyster for signatures.

Multiple LAHC team members will reply within 24 hours. NOTE:  Please do not come to Lyster for signatures. Separation Health Physical Exam (SHPE), Medical Out Processing Instructions for Retirement, ETS or Separation:  Use individual links identified in each step (Steps 1 thru 5). National Guard/Reserves coming off of Active Duty (AD) Orders, ONLY Steps 1 and 2 is mandatory, unless you have been seen and have medical care established at LAHC while on AD. Step 2. Step 3. Soldier is within 30 days of separation or terminal leave.

Step 4. Soldier calls Central Appointment Line at 1-800-261-7193, option. 1. They are available Monday – Friday 0700-1600 and Soldier requests Part 1 appointment for a separation physical.

Step 5. Soldier attends and completes the Part 1 separation physical and then will leave with an Appointment for a Part 2 Separation Physical with PCM to finalize the physical.

Clinic Hours:

Monday-Friday, 0730-1615; closed weekends, holidays, and every 3rd Wednesday at noon. NOTE: Lyster is NOT an ER/Urgent Care facility.

Sick Call (Active Duty Service Members with Acute Issues Only): 

Monday-Friday, 0600 - 0630; closed weekends and holidays.  NOTE:  Acute injuries and illnesses ONLY. Anything older than 72 hours, please make an appointment with your provider.

Pharmacy Services

All prescriptions and OTC requests should be submitted to the pharmacy by filling out a drop off form. Please see our website and FaceBook page for instructions and up to date information. Drop off forms can be found on the Lyster website and on TOL under the Appointments button. Drive-thru hours are Monday-Friday 0730-1615.

Central Appointment Line 800-261-7193, option 1 for virtual, face to face or specialty appointment requests, telephone consultations or Provider messages. Monday – Friday from 0700 to 1600.
Tricare Nurse Advice Line (NAL):

1-800-874-2273, if the clinic is closed or you are out of the Lyster area, traveling or on leave. The NAL will determine your place of care; ER, Urgent Care (UC) or schedule appointment at Lyster. *** Mandatory for Active Duty Service Members to call 800-261-7193, option 1 and report ER or Urgent Care visits.

Medical Release of Information: If your dependents are 14 and older, they must have a signed DD Form 2870, Medical Release of Information on file with LAHC Patient Administration/Medical Records for spouses, parents, or guardians to access medical record or appointments for their dependents.
